Billion describes his friend as "an artist." Hussain says his skill in surgery comes because his right brain is dominant, and he thinks in three dimensions.
A 57 year old woman failed to show up for her appointment for surgery. When he called to ask why and demand that she keep her appointment because, as he says, "I like being the sheriff" she told him relatives had said she was too old.
A 20 year old unmarried woman from a Pakistani tribe came to see Hussein. He knew the deformity was the reason she was unmarried at an age when her peers already had wed. He refused to pass her on to a student, insisting on handling the surgery himself. As he describes it, "I knew that I was going to make magic."

Hussain can tell story after story after story about the people he's helped, both in the United States and elsewhere. While he describes himself as an "in your face kind of guy" and as someone who has made enemies among other physicians, it's obvious that he is beloved by those he helps.

"Why did I do three a day? Because nobody else could do it. Also, deep down, I wanted to make an impact. My idea is to let everybody know good can be done, and you have to show it in images, images you cannot forget."
A Pakistani philanthropist funded Cleft Hospital in Gujrat, Pakistan. Hussain spends much of his time there, teaching other surgeons how to repair cleft lips and cleft palates. He has been asked why he goes overseas, and the answer is simple.

Not everyone thought Sioux Falls needed a plastic surgeon.
Hussain changed the life of a man he calls the "monkey wallah," a 47 year old man who traveled from village to village, putting on a show with his trained monkey. All he asked in return was that the man put on a show for hospitalized children.
"Parents love him, and kids," Billion says. "Some nurses and doctors are afraid of him. He has a heart that's gigantic in terms of its kindness. He does your surgery because you need surgery. These are kids in dire need of a surgeon who has the ability to create a beautiful result out of a very significant deformity."
When Hussain says such things, he's not bragging. Well, OK, maybe he's bragging a bit. But he does it with a mix of what his friend Dr. Jack Billion describes as an air of supremacy that overlays a heart of gold.
Pakistan to repair cleft palates and lips.
Hussain, born in a region now part of Pakistan, came to South Dakota in 1975. He considered an internship at Stanford University in California but after visiting a friend here, he realized he was tired of living in urban settings.
"Some physicians were very encouraging to me, but most said there was no need for a plastic surgeon," Hussain says. "I stayed anyways. I borrowed money from First National Bank."
He became a surgeon in obedience to his father's lifelong dream for him. He was 16 years old and a senior in high school.
In 2003 he left his independent practice and joined what is now Sanford Health. In 2011, Sanford asked him to become part of its world clinic. Four times a year he travels to India and Reebok Lifting Shoes
The first patient he operated on here came back to see him 15 years later. After her face was repaired, she left an abusive relationship and graduated from nursing school. She brought her husband to meet Hussain, the husband explaining that his wife wanted him to meet "the most important person in her life."
"There is a need, and I know I can make a difference, and I made a difference," Hussain says. "How did I make a difference? I made a difference by doing the surgery and showing the local surgeons what is possible if you do it right."
After her incisions healed, she told Hussain, "In 57 years, for the first time I smiled, and I learned the value of what a smile is."
